Health insurance plans we accept

We accept most health insurance plans even if we are out-of-network. If you have out-of-network benefits, we will accept your plan.  If  you have an HMO or EPO with no out-of-network benefits,  we will request an in-network waivers (also called a Gap Exception). This is done through the prior authorization process.

Our sports medicine team provides specialized care for athletes of all ages and skill levels. We focus on injury prevention, rehabilitation, and performance enhancement.

Our licensed physical therapists provide personalized care to help you recover from injury or surgery. We use evidence-based techniques to help you regain strength, mobility, and function.

Since we are in-network with our local BCBS plans, we are in-network with all of the BCBS plans across the country. Including Anthem BCBS.

Only certain areas, we will need to verify your Aetna Health plan

All areas as there is a Federal Law for active military under the medical nutrition equity act. 

We are out-of-network with most Medicaid plan, this requires a in-network waiver and single case agreements through prior authorizations.

We are pending our application and enrollment process with Medicare and will update this site once we are enrolled or not.
