Health Insurance Plans: In-network and Participating
In general, we accept most health insurance plans even if we are out-of-network. If you have out-of-network benefits, we will accept your plan. If you have an HMO or EPO with no out-of-network benefits, we will request an in-network waivers (also called a Gap Exception). This is done through the health plans provider prior authorization process.
Horizon BCBS of New Jersey
All Commercial Blue Cross Blue Shield plans
Since we are in-network with our local BCBS plans, we are in-network with all of the BCBS plans across the country. Including Anthem BCBS.
Highmark BCBS of Pennsylvania

Only certain areas, we will need to verify your Aetna Health plan
Tricare
All areas as there is a Federal Law for active military under the medical nutrition equity act.
Champ VA

Out-of-state Medicaid Plans and MCO Plans
We are out-of-network with most Medicaid plan, this requires a in-network waiver and single case agreements through prior authorizations.
